Walk-ins Only
All Customers Must Arrive with:
- DS-11 Application 04-2022 version “completely filled out”, written in “black ink”, “mother’s maiden name” on 2nd page, line 10, AND “with no errors”
- All necessary documents & photocopies
- Separate the documents for each member of your party
- Passport Photos
- Check or money order for passport payment
- Credit or debit card for appointment payment
- We will not execute hand-carry applications or letters from the passport agency
What to bring to the Passport Fair
First Time Applicant, You Will Need:
1. DS-11 Application, version 04-2022 that is completely filled out, in black in,k and without errors (with the mother’s maiden name in the parental section on page 2, line 10)
2. Birth Certificate or a Citizenship/Naturalization Certificate and a Photocopy
3. Social Security on the application
4. Passport Photo
5. Check or Money Order to the U S Department of State
6. Credit or Debit Card for appointment payment
7. Identification (driver’s license, US Passport Book/Card, Military I. D.; Government I.D., or a Foreign Valid Passport & a copy of the front and back of the I. D.). Will need a front and back copy of both parent’s I. D., if the kids are 15 years old or younger.
8. Both parents must be present if the kids are 15 years old or younger (A non-appearing parent must submit a notarized form, DS-3053, and a copy of the front and back of the driver’s license).
9. One parent must accompany a 16 or 17-year-old (the parent must submit a copy of the front and back of the I.D.)
10. Separate the documents for each member of your party (to present to the agent separately)
a. DS-11 Application
b. Birth Certificate or Citizenship Certificate and a Photocopy
c. Passport Photo
d. Check or Money Order for the Department of State
e. Original I.D. and a Photocopy
f. Supporting Documents (if applicable)
g. Remember to bring a debit or credit card for appointment payment
If You Are Renewing the Passport Book/Card, You Will Need:
1. All of the above EXCEPT number 2.
2. If you are 16 years old or older, you will bring the Passport Book/Card.
3. If you are 15 years old or younger, you will bring the Passport Book/Card AND a photocopy of the birth certificate.
4. If you have lost the Passport Book/Card, bring the Birth Certificate or a Citizenship/Naturalization Certificate and a Photocopy.
